When a teacher requests a signature on a PDE 338P form so they can get their Level II Certification the following steps are followed:

1. The form is received for signature.
2. The form should always be submitted with Sections I, II & III filled out by the teacher – even though it says for the school to do it.
3. The teacher’s file is located at OCE.
4. OCE will confirm that the teacher achieved Level I Induction.
5. Once Level I is confirmed the form and a cover letter is given to the Secretary of Elementary Education for signature.
6. If proof of Level I is not in the file or there is no file found in OCE a request will be sent to the teacher asking for the signature and to look at their personal files to see if they have a copy of a letter or certificate from OCE congratulating them on achieving Level I Induction.

NOTE:  Sign off on Level II Induction must be supported with proof of completion of Level I Induction.

NOTE:  If documentation cannot be produced as proof of completion of Teacher Induction Level I the process will need to be repeated.
