Documents Needed
Appendix A – these are kept in the teacher’s file at the school – do not send to OCE.
Appendix B – there are 2: (1) one in the Fall and (2) one is usually done in the Spring. The first one should be sent to OCE when it is completed.
Appendices C, D & E – once completed they are to be sent with the second Appendix B to make a complete packet.
Procedure Followed at OCE
When the first Appendix B is sent in, the teacher’s file is pulled and it is moved to a file “Pending Inductions” and waits for the rest of the paperwork to be sent down later in the year.
If the teacher file is not found at OCE because the teacher was hired at the parish level and paperwork was not forwarded to OCE, a file will be made up for that teacher.
Confirmation that the teacher attended the Archdiocesan Induction Training, held every August, by checking the sign-in sheets.
The teacher file at OCE will be checked for a signed card as proof that they attended their induction training.
After making sure all the appendices are accounted for and filled out properly the paperwork/teacher file is sent in to the appropriate Assistant Superintendent for review and to be sure all the criteria have been met.
Upon approval, the paperwork is returned to Jeanne Steitz for completion.
A cover letter and certificate is prepared for both the teacher and the teacher’s mentor for signature.
Final signature is obtained from the Secretary of Elementary Education.
Finalized paperwork is mailed to the principal of the teacher’s school.
Finalized paperwork is put into the teacher’s file and filed.
The teacher, once presented with the Certificate, should always make a copy and keep it with their personal files.
