Terra Nova Testing – Accommodations Forms

Accommodations for standardized testing should only be used if the same accommodations are being afforded to the students on a regular basis.  Some schools choose not to accommodate in order to get a more accurate representation of where the students place in relation to the rest of the class.  If you choose to use accommodations:

  • All requests for accommodations should be completed and submitted by November 29. These can be accessed from the Policy CIA – 609 web page.
  • There are three forms listed:
    1. Elementary Curriculum Accommodations and Adapted Grading Parental Agreement Form
      This form should be completed at the beginning of the school year and kept in the student file after all signatures have been received.
    2. Elementary Curriculum Accommodations and Adapted Grading Parental Plan Form
      This form records the plan for the student requiring accommodations.  Three copies are made — one for the parent, one for the teacher and one for the school file.
    3. Elementary Curriculum Standardized Testing Accommodations Request Form
      This is a single sheet completed after the previous forms are completed. This form should be submitted electronically as either a pdf document or a Word document. Please do not submit as a Google document as these are often difficult to open and sign electronically.

      • By submitting this document you are verifying that the following has been completed:
        • The student has been evaluated within the past three years and has been determined by a school psychologist/child psychologist to be in need of accommodations for testing.
        • You have procured parent consent.
        • A plan has been developed in keeping with the recommendations made.

Accommodations are not made without the proper testing and documentation.  Parents and teachers cannot make requests for accommodations unless the proper procedures are followed.

Level 3 accommodations are for Special Education students only.  Level 3 requires the use of assistive and adaptive devices found in the Schools of Special Education.

Because of distancing required by COVID protocols, we are asking that all forms be submitted electronically so that we can process them with a quick turn-around.  Our time in the office is limited and mailed forms may sit in waiting for a long period of time.

Instructions for completing the Terra Nova Spreadsheets for labels will be sent once they are received.
